Nursing Home Transition & Diversion (NHTD)
Live life on your terms — outside of institutional care.
This Medicaid program supports seniors and individuals with disabilities in transitioning from or avoiding nursing home care by offering community-based support.
Personal Care Assistance (PCA) Program
Helping hands for everyday independence. Through PCA, individuals receive help with daily tasks like bathing, dressing, and meals — provided by trained caregivers in the comfort of their own home.

My name is Maureen and Mr. Mode EDOTIMI is my ILST for over 6 months. He represents Priority Cares.
